予約について
予約はお電話にて受け付けております。下記電話番号からおかけください。
来院に必要なもの
保険証・医療証を必ずご持参ください。なお保険証・医療証がない場合は、保険診療ができません。
また、普段飲んでいるお薬やサプリメント、血液検査や健康診断などの検査記録がありましたら、診察の際にお見せください。
当院は、院内・院外処方をご用意しております。
ジェネリック医薬品をご希望の方は、どうぞご遠慮なく医師またはスタッフにお伝えください。
Upon your first visit to our Clinic
- Please kindly confirm the expiration date of your Insurance/Medical Card. If you have an expired Insurance/Medical Card or do not have an Insurance/Medical Card, the medical fees must be self-paid.
- If you are on traveler’s insurance, you will have to pay your medical fees in advance. Please ask your insurance company for reimbursement.
- Please call us beforehand for reservations.
外国の方に向けての注意点
期限等を確認し、有効な健康保険証をお持ちください。
期限切れ、保険証をお持ち出ない方は、自費での診療となります。
また、旅行用の保険に入られている方は、一旦料金をお支払いいただき、帰国後、保険会社に請求していただくかたちになります。

To our foreign patients
- Please bring your Insurance/Medical Card. If you do not have or fail to bring your Insurance/Medical Card, we cannot treat you under the health care system.
- If you have any daily medication and or any test results regarding your health, please bring them with you when you visit us.
- We have both internal and external prescription of drugs. We can also prescribe generic drugs upon your request. Please do not hesitate to ask the doctor when you visit us.
